Join a dedicated healthcare team as a Nurse Practitioner in a primary care outpatient clinic, focusing on providing value-based care to adults on Medicare. This permanent position in Portsmouth VA offers the opportunity to work collaboratively with a dedicated care team to ensure patients receive comprehensive and coordinated healthcare services.

Job Details

• Employment Type: Permanent
• Hours Per Week: 40
• Schedule: Monday - Friday, 8 AM - 5 PM
• Salary: $105,000 - $135,000/year (Depending on Experience)

Job Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
• At least 1 year of experience in primary care: internal medicine, geriatrics, or family medicine preferred

Responsibilities

• Provide exceptional primary care in a clinical setting
• Conduct office visits for routine and acute issues
• Administer annual wellness visits and health risk assessments
• Coordinate care with other healthcare providers, specialists, and facilities
• Lead population health initiatives in coordination with the Care Team
• Assist with phone triage and patient outreach
• Educate patients on health conditions, care plans, and treatments
• Participate in promotional activities and conduct home visits as needed
